I understand that my entry is subject to rejection at any time (including during the Championship by the ASGA or the Sage Meadows Country Club.  The reason for rejection may include unbecoming conduct.)   Any questions shall be settled by the ASGA, whose decision is final.  I hereby acknowledge, upon submitting this entry form,  that the Arkansas State Golf Association and host club are not responsible for personal injury during this championship.  I have read the USGA Rules of Amateur Status and conform with those rules in every respect.

40TH ASGA Men's Match Play Championship

ELIGIBILITY: This championship is open to male amateur golfers who are bona fide residents of the state of Arkansas (also see ASGA Stroke Play) and a current individual member of AS GA. Participants must have a USGA handicap index of 8.0 or less.

FORMAT: Contestants will play 18 holes Thursday, July 20 to determine the 64-man bracket.   is the defending champion and automatically seeded first. There are 63 spots available for match play. Two matches will be played each day Friday and Saturday, morning and afternoon. The semi-finals will be Sunday morning and the finals will be Sunday afternoon.

PRACTICE ROUND:  Sage Meadows C.C. will be open for a practice round on Wednesday, July 19. Players must make a tee time reservation (479-571-3673) and pay a cart fee to playa practice round..

CARTS/CADDIES: Players must make their own arrangements for caddies. Players desiring the use of rental carts must make reservations on the entry form. The ASGA is committed to furnish carts only to advance reservations.

ENTRY FEE/DEADLINE: The entry fee is $100.00 per player plus $3.00 transaction fee for online entry and must accompany the application.   The deadline for all entries to reach the ASGA office is Monday, July 11. You may also enter online at www.asga.org with a

PRIZES/GIFT CERTIFICATES: The champion will receive custody of the Roy Wood trophy for one year. The champion and runner-up will also receive individual trophies.  The top 16 finishers will receive merchandise certificates from the StoneBridge  Meadows G.C.
